DOT Hydro Testing Locations by State

Sorry for the bump but I must be missing a piece of information...

I've located a bunch of testing facilities near me here in Houston but the website just lists a bunch of codes for what is tested at that facility. What is the "Specification" code for hydro-testing carbon fiber tanks? I've been scouring the web for the answer but have come up short.

There is a # on the tank and I believe it is 10915.

I'd call any with Fire Equipment in their name.  Also, this specifically lists the 10915:

The RIN/VIN Tester link with the interactive map doesn't pull up anything for me even when I click the All button.

I did click the link near the top "to generate and display search results in a tabular format" and that works, but I didn't find anybody in Virginia who could do the hydro. Am I doing this correctly?

My Great White 4500 psi carbon fiber tank is going on 4 years old so I want to know where I can get it hydro'd in advance so I am prepared for when the time comes. I'm under the impression it needs hydro testing every 5 years. Right?
