Have you looked at vented set screws as a possibility and open up the vent hole size to fit?
post a basic drawing, odds go up.. also , take note that if the threads are tapered, its actually a 1/8npt

How long does it need to be? If it's only ~3/16, a socket jam screw from Fastenal or ACE might be the easy way to go. For somebody to fab one would take a bit of time. First chuck up a bit of stock, then drill and tap a blind hole, THEN drill the set screw. Making the jig takes longer than drilling the screw.
Ehh... Probably like between 1-1/4 to 1-3/4 otherwise I'd do just that. I have a polypropylene Hose barb threaded for 1/8 npt, not ideal because of aesthetics and limited adjustability, but it threads into the adjuster body well enough to at least keep the experimentation going. If HDs suggestion of McMaster carr don't Pan out, I may be better off buying a whole adjuster and doing the drilling myself

It wouldnt even be a concern if I had so much as a drill press. I'm usually pretty good improvising these kinda things tho.

Do you have the 3/8-24 bolts? What is the overall length? What is the ID that you need? Do you need a through hole? What finish is required in the hole? Do you need it to be a highly polished hole? What sort of precision are you looking for? Do the bolts need to be high strength? I just have a mini lathe, but it sounds like it would be up for this job.

This doesn't need to be anything fancy, length between 1 1/4 to 2" can work, I'll likely be fine tuning the final hole dia. Myself. Brass is preferable, but not necessary. It is going to function exactly like a hammer spring adjuster, except instead of a spring guide in front it will have a Rod going through it to function as a cocking link and ssg, if you're familiar. Just needs to be functional, not fancy
